2025 Statement of Benefits (T5007) are available online for Injured Workers who had a MyWorkplaceNL account before February 14, 2026. T5007 forms available online will not be mailed; you can download, save, or print them. If you do not have a MyWorkplaceNL account your T5007, will be mailed to you.
